Based in San Francisco, JotForm is the leading online form building platform that supports healthcare organizations, meeting all U.S. Health Insurance Portability and Accountability Act (HIPAA) regulations.

When you use the webforms on our site to request an appointment or refer a patient, the data you enter will be encrypted and protected on HIPAA-certified servers and databases. The JotForm service that we use is not only HIPAA compliant, it is PCI DSS Service Provider Level I compliant, which is the highest bank-level security available. Only users authorized by Westford Endodontic Care will have exclusive access to submission data.
